Customer Application

Required Documents

TABE Schedule

1. Register on This is mandatory.

2. Complete the Customer Application using one of the ways outlined below.

3. Submit the Required Documents

4. Schedule a time to take the Test for Adult Basic Education (TABE) by calling a netWORKri Career Center.
    This test will help us determine your math and reading levels to better assist you.

You will be contacted by a Career Counselor for an appointment once we have received your application and documents and you have taken the TABE


Ways to complete and submit the customer application:

1.  Complete Customer Application online at any time, from any computer, and save it as a file.

      • Email the completed application and required documents to the office location of your choice:
West Warwick:

These email addresses are for application/documents submission only. Emails not containing an application or documents will be discarded.
      • If you are unable to attach your documents with your application, bring it at any time to the office location you selected. A locked drop box is available for you to place your copies during regular business hours. Copy machines are available if needed.

2.  Complete Customer Application at the Self-Serve Application Center located in each office. Each Application Center offers instructions, use of a computer and printer, as well as paper appplications.


3. If you are unable to complete the application using one of the two above ways, you may stay after the TABE for help from a staff member.

Required Documents:

Submit a copy of one document from each category with your application to allow us to verify your information. 

Proof of Age (submit one)

- Valid Driver’s license
- Valid State-Issued ID
- U.S. Passport
- U.S. Birth Certificate
- DD214

Work Authorization (submit one)

- U.S. Passport
- U.S. Birth Certificate
- Alien Registration Card
- Naturalization Certificate

Proof of your SSN (submit one)
This MUST BE provided at first meeting with career counselor

- Social Security Card
- W2 (if full SSN is shown)
- DD214

The below documents are only required if they apply to you:

If you are collecting Public Assistance (submit one)
Must be dated within the last 6 months and show your name

- Most Recent SNAP Award letter
- SSI Award Letter
- Other Public Assistance Records

To be excused from TABE Test (submit one)

- Copy of Bachelor’s Degree
- College Transcripts showing
   a Bachelor’s degree or higher

If you are a Veteran (submit one)

- DD214
- Military Records

Proof of Residency (submit one)
 if Driver's License Address is different from current address

- Anything showing your name    and current address

Schedule a TABE Test:

Call the networkri office you have selected for counseling to schedule the TABE.

TABE tests are held weekly with the below schedule:
